Finding us, parking, and what to expect.
Please arrive about 15 minutes early. That leaves time to park, find the office, and settle before your appointment begins.

Our office is on the second floor at 475 East Main Street, Suite 201, Patchogue, NY 11772, conveniently located in Patchogue.
Office hours are Monday – Friday, 9:00 a.m. – 5:00 p.m.. If you have trouble finding the building or the parking area, call (631) 366-3369 before your appointment and we will talk you in.

Free parking, adjacent to the building
There is a parking lot next to the office, and parking is free. Designated accessible spaces are available near the building entrance.

On the day
What to bring
A few of these matter clinically, not just administratively — glasses and hearing aids in particular, since a test of memory should not accidentally become a test of eyesight.
Photo ID
For check-in.
Insurance card
The physical card, or a clear photo of both sides.
Medication list
A written list, or simply bring the bottles — either is fine.
Glasses and hearing aids
Bring and use whatever you normally rely on to see and hear.
Referral or authorization
If you have it in hand. If your physician faxed it, we already have it.
Prior records
Any previous evaluations, imaging, or specialist reports we have asked for.
Preparing
Before your appointment
Testing measures your best level of functioning, so a few practical things genuinely change the result.
- Get a full night's sleep if you can. Fatigue affects exactly the abilities being measured.
- Eat a normal meal beforehand. Testing runs several hours.
- Take your medications as usual, unless your prescribing physician or Dr. Barnoski has told you otherwise. Do not stop anything on your own.
- Avoid alcohol and recreational drugs for at least 24 hours beforehand.
- Wear comfortable clothing, and bring a snack, lunch, and a drink if you would like.
- If you are unwell — fever, vomiting, anything contagious — call us before coming in.
During testing
What to Expect
A neuropsychological evaluation is tailored to the reason for referral and your individual needs. The process begins with a consultation to discuss your concerns, history, and the reason for referral. If testing is appropriate, it is scheduled for a separate day and may assess areas such as memory, attention, learning, language, problem-solving, and other aspects of cognitive functioning.
The amount of testing needed varies depending on the referral question and the nature of the evaluation. Breaks are provided as needed during the testing appointment.
A family member or caregiver is welcome to accompany you to the consultation and, when appropriate, may be asked to provide additional background information. Formal testing is completed individually with the patient to help ensure accurate and reliable results.
We ask that you please silence your phone and other personal devices during testing.
Accessibility and accommodations
We are committed to providing accessible care. If you need an accommodation because of a disability, please tell us when scheduling so we can arrange it properly in advance.
Common accommodations include wheelchair-accessible access, assistance for mobility limitations, seating for a family member or caregiver, and extra time at check-in. Please bring any assistive devices you normally use.
If you need help communicating because of hearing, vision, language, or other needs, let the office know before your appointment so we can work out the best arrangement.
