Referrals and getting started

Do I need a referral?

Yes. We require a letter of medical necessity or an office note from your physician that explains the need for a neuropsychological evaluation. This documentation is needed to begin the authorization process.

Can my physician refer me directly?

Yes. Physicians may fax a letter of medical necessity or an office note requesting a neuropsychological assessment, along with any relevant medical records, directly to our office at (631) 366-2043.

How do I schedule an appointment?

Start by requesting an evaluation through this website. The online form is the quickest way to provide the information we need to review your request. Once your information has been reviewed, our office will contact you regarding scheduling. If you prefer to call, our AI phone assistant can collect the information needed to begin your request and forward it to our office for review.

What happens after I submit my request?

Our clinical staff reviews your information and the reason for the referral, confirms your insurance benefits, and determines whether a neuropsychological evaluation is appropriate for the referral question. Once the review is complete, our office will contact you regarding scheduling.

How soon can I be seen?

Availability varies with the reason for evaluation, insurance authorization, and the schedule. Our office will contact you with the earliest available appointment once your intake has been reviewed. You may also ask to be contacted if an earlier appointment opens up.

About the practice

What is a neuropsychological evaluation?

A comprehensive assessment of thinking and brain functioning. It evaluates memory, attention, language, problem solving, executive functioning, processing speed, visual-spatial skills, and emotional functioning, in order to clarify a diagnosis and produce treatment recommendations.

What conditions do you evaluate?

ADHD, learning disabilities, autism spectrum disorder, memory loss and dementia, traumatic brain injury, concussion, stroke, brain tumors, epilepsy, multiple sclerosis, Parkinson's disease, cognitive decline, medical conditions affecting cognition, and emotional or psychological conditions where cognitive concerns are present.

What ages do you evaluate?

We evaluate children aged 7 and older, adolescents, adults, and older adults.

Is Dr. Barnoski board certified?

Yes. Dr. Barnoski is board certified in Clinical Neuropsychology by the American Board of Professional Psychology (ABPP), and has decades of experience evaluating complex neurological and psychological conditions.

Do you provide therapy or counseling?

No. Our practice focuses on neuropsychological evaluation and consultation and does not provide therapy or counseling. If another type of service would be more appropriate for your needs, we will let you know and help direct you to the appropriate type of provider.

Insurance and payment

Do you accept my insurance?

We participate with several plans, including Medicare, UnitedHealthcare, Oxford, EmblemHealth, HealthFirst, Empire / NYSHIP, GHI, New York State Workers' Compensation, and No-Fault. Insurance participation changes periodically, so our office verifies eligibility before your appointment.

Will you verify my benefits?

Yes. We verify your benefits as a courtesy, and can often do it electronically within seconds. However, benefit verification is not a guarantee of payment. You remain responsible for any deductibles, copayments, coinsurance, or non-covered services determined by your insurance company.

What will it cost me out of pocket?

Your out-of-pocket cost depends on your specific insurance plan and benefits. Once your benefits have been reviewed, our office will explain your estimated financial responsibility whenever possible. Please keep in mind that benefit information is not a guarantee of payment, and your final responsibility is determined by your insurance plan.

What if my insurance does not cover testing?

Private-pay options are available, and some insurance plans may provide out-of-network benefits. Our office will review the applicable fees and available insurance benefits with you before an evaluation is scheduled so you understand the anticipated cost in advance.

When is payment due?

Copayments, deductibles, coinsurance, and private-pay fees are due at the time of your appointment.

The evaluation

How long does testing take?

The length of an evaluation varies depending on the referral question and the testing needed. The evaluation includes a clinical interview with Dr. Barnoski and several hours of testing, which are scheduled separately. Our office will provide additional information about what to expect when your appointments are scheduled.

What should I bring?

Photo ID, your insurance card, a list of your current medications (or the bottles themselves), your glasses and hearing aids if you use them, your referral or authorization if applicable, and any records or prior evaluations we have asked for.

Should I stop taking my medications?

No. Continue taking your medications as prescribed, unless your prescribing physician or Dr. Barnoski specifically instructs otherwise.

Can someone come with me?

Yes. A family member or caregiver may accompany you and may be asked to contribute history during the interview. Family members are not present in the room during formal testing.

Will the testing be difficult?

Some parts of the testing may feel challenging, which is expected. The tests are designed to assess different areas of thinking and cognitive functioning. Breaks can be provided as needed, and the testing is not painful or invasive.

Results and reports

When will I receive my report?

Reports are typically completed within one to two weeks after testing is finished, although timing may vary depending on the complexity of the evaluation. At the evaluation, you will be advised that you may call the office after one week if you would like to schedule a follow-up appointment with Dr. Barnoski to review the results and receive a copy of your report.

Will someone go through the results with me?

Yes. You can either review the results with your referring physician, or schedule a feedback appointment with Dr. Barnoski to go through the findings and recommendations. Results are not given out over the phone by staff.

Will my doctor receive a copy?

Yes. The referring physician receives a report once it is complete.

Can the report be sent to my attorney, school, or employer?

Yes, with your written authorization.

Policies

What is your cancellation policy?

We ask for at least 24 business hours' notice when canceling or rescheduling a consultation and at least 48 business hours' notice for an evaluation, since testing appointments reserve a substantial amount of time. Late cancellations may result in a cancellation fee and may affect our ability to reschedule future appointments.

What happens if I am late?

Arriving late may shorten your appointment or require it to be rescheduled, depending on the day's schedule. Please call as soon as you know, with an estimated arrival time, so we can work out whether the appointment can still go ahead.

What happens if I miss my appointment?

We maintain a strict no-show policy: an evaluation reserves several hours that cannot be reallocated at short notice. Patients who miss an appointment without notice are generally not rescheduled. If an emergency prevented you attending, tell us and the office will review it.

What should I do if my situation is urgent?

If you are experiencing a medical emergency, call 911 or go to the nearest emergency department. This practice does not provide emergency or crisis services.
